We are seeking a highly motivated and compassionate Hospice Agency Liaison to join our team in Southern California. As a Hospice Agency Liaison, you will be responsible for developing relationships with healthcare providers, hospitals, and other healthcare organizations to promote our hospice agency services. You will be the primary point of contact for our agency and will work closely with patients and families to ensure they receive the highest quality of care.

Responsibilities:

  • Develop and maintain relationships with healthcare providers, hospitals, and other healthcare organizations
  • Promote hospice agency services to potential referral sources
  • Conduct on-site visits to healthcare facilities to educate staff about hospice services
  • Collaborate with the hospice team to develop and implement care plans for patients
  • Educate patients and families about hospice services and provide emotional support
  • Participate in community events to promote hospice services

Qualifications:

  • Minimum of one year of experience in healthcare sales, marketing, or customer service
  • Excellent communication and interpersonal skills
  • Compassionate and empathetic attitude towards patients and families
  • Ability to work independently and as part of a team
  • Knowledge of CRM or other software applications for tracking and monitoring referrals and relationships is preferred
  • Knowledge of hospice regulations and Medicare/Medicaid reimbursement is preferred
